Clinical pharmacology

Isoflurane is an isomer of enflurane and is an inhaled anesthetic with rapid induction and recovery of anesthesia. There is no sympathetic nervous system excitability during anesthesia, which can make the heart slightly sensitized to adrenaline and have a certain muscle relaxation effect. This product has a low metabolic rate in the liver and is therefore less toxic to the liver.

【Dosage】

Nebulizers for inhalation of isoflurane are rigorously calibrated to enable accurate control of the concentration of the anesthetic that is administered. The MAC value of isoflurane varies with age, and the average MAC values for different age groups are as follows:

1. Anesthesia induction: It is recommended that the initial inhalation concentration be 0.5%, and gradually increase to 1.5-3.0% in 7-10 minutes, that is, enter the anesthesia period.

2. Anesthesia maintenance:

Surgery: It can be mixed with 1.0-2.5% isoflurane and oxygen/nitrous oxide mixed gas. If it is mixed with oxygen alone, the concentration of this product should be increased by 0.5-1.0%.

Caesarean section: When mixed with oxygen/nitrogen oxide mixed gas, the concentration of this product is 0.5-0.75%.

【Adverse reactions】 

Occasionally, the arrhythmia increases and the number of white blood cells increases. Coughing and irritation of the throat during induction can cause respiratory depression and hypotension; chills during resuscitation, nausea and vomiting.

[taboo】

1. It is forbidden to be allergic to this product or other halogenated anesthetics.

2, the use of this product after the occurrence of malignant hypercarbemia is banned.

【Precautions】

1. The depth of anesthesia with this product is very easy to change. A nebulizer should be used to accurately set and control the drug output.

2, this product has an inhibitory effect on breathing, so preoperative medication should be based on the specific circumstances of the patient. Generally, anticholinergic drugs are used.

3. Like other halogen anesthetics, isoflurane can cause blood pressure drop and respiratory depression, and pay close attention to changes in blood pressure and respiration.

4. The significant decrease in blood pressure during anesthesia maintenance is related to the deepening of anesthesia. This phenomenon should reduce the inhalation concentration.

5. Increased intracranial pressure should be used with caution.

【Pregnant women and lactating women】Pregnant women are prohibited (except for caesarean section).

According to reports in the literature: This product is relatively less metabolized in the human body, only 0.17% of the inhaled amount is excreted from the metabolites in the urine after surgery. The concentration of inorganic fluoride in plasma is generally less than 5 μmol/L 4 hours after anesthesia and returns to normal within 24 hours after anesthesia.
