Clinical pharmacology

Orlistat is a reversible gastrointestinal lipase inhibitor that exerts a therapeutic effect by inactivating a covalent bond with gastric lipase in the stomach and small intestine and the active serine site of pancreatic lipase. Inactivated enzymes do not hydrolyze fats (mainly triglycerides) in foods into absorbable free fatty acids and monoacylglycerols. Undigested triglycerides are not absorbed by the body, reducing calorie intake and controlling body weight. The drug does not need to be absorbed through systemic absorption.

Dosage

When the meal contains fat, take 60 mg /120 mg (1 capsule) orally at mealtime. Do not exceed 3 capsules per day.

【Adverse reactions】

4. Rare reports of allergies to this product, the main clinical manifestations are itching, rash, urticaria, angioedema, bronchospasm and allergic reactions, the emergence of herpes is very rare.

5. It has been reported that in patients who have been combined with orlistat and anticoagulant, a decrease in prothrombin and an increase in the International Normalized Ratio (INR) have occurred.

6. The use of orlistat has been reported to be a rare increase in aminotransferase, elevated alkaline phosphatase, and severe hepatitis, and cases of liver failure, some of which require liver transplantation or can directly lead to death. Post-marketing drug monitoring found reports of pancreatitis, and the causal relationship between pancreatitis and orlistat has not been established.

11. Orlistat may increase the risk of urinary crystallization in the patient. Patients with a risk of renal insufficiency should monitor renal function during the course of taking orlistat. Patients with a history of high oxalic acid and calcium oxalate kidney stones take orlistat. Be cautious.

12. Due to rare acute liver cell necrosis or severe liver injury after orthostroemia, some patients need liver transplantation or can directly cause death, so the prescription doctor should guide the patient to take the initiative to report the use of Orlistus.
